Clinical pharmacists are important members of the multidisciplinary team within primary care networks (PCNs), which were established across England in July 2019. Our Pharmacists take responsibility for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly, people in care homes, those with multiple co-morbidities (in particular frailty, COPD and asthma) and people with learning disabilities or autism.
First Contact Physiotherapists
First contact physiotherapists are important members of the multidisciplinary team within primary care networks (PCNs), which were established across England in July 2019. Our physiotherapists are able to assess new and existing musculoskeletal problems, including diagnosing conditions, offering physiotherapist advice, arranging for further imaging and referring to the hospital hub for further review.

There is a private room next to reception should you wish to discuss a private, confidential or sensitive matter with any member of staff. Please ask at reception if you wish to use this facility. Our reception team are here to help and support you.
